Most urologists have contracts with the various insurance companies and the cost is determined by that. Most commonly the expense is between $750.00-$1,200.00. If you don't have insurance or you have a high deductible, most urologists will perform the procedure as if you did have insurance and use what would customarily be the allowed amount.

Check with your insurance. For the past 35 years, they determine the cost of everything a physician does. Whatever they tell you is the way it is. You may have out-of-pocket co-pays or deductibles you may need to pay up front before your insurance pays the rest, if they pay anything. If you have no insurance, the doctor's fee is what the out-of-pocket cost will be for you.
