Should I have another vasectomy?

I had a vasectomy with excision, both ends cauterized, and fascial interposition. Sperm were then found in the first semen test 15 weeks and 20+ ejaculations later.

No. You have not reached enough ejaculations to analyze the success of the vasectomy. It takes around 35 ejaculations or at least 3 months before we really know the effectiveness of the vasectomy.
