Should I have another vasectomy?
I had a vasectomy with excision, both ends cauterized, and fascial interposition. Sperm were then found in the first semen test 15 weeks and 20+ ejaculations later.

No. You have not reached enough ejaculations to analyze the success of the vasectomy. It takes around 35 ejaculations or at least 3 months before we really know the effectiveness of the vasectomy. If the vasectomy has been done properly then there is very little chance that she will have sperm beyond the above
