Based on this semen analysis, am I sterile?

I had an open-ended vasectomy in August 2018. The first semen sample in November 2018 had 0-2 dead sperm. The second sample in Jan 2019 had 0-2 dead sperm. Now the third sample in April 2019 showed 0-2 dead sperm. I'm not sure if I'm statistically sterile yet. What are the changes I could impregnate my wife?

You are "statistically" sterile, however, most patients and urologists prefer to have an absolute negative semen analysis before unprotected activity. Studies have shown that situations like yours are fairly common and very unlikely to result in pregnancy.

Having said this, the concern I have in my patients is could this represent the very earliest phase of recanalization?

Your options are to continue to check until completely negative, go ahead and pursue unprotected sex, or have the procedure repeated. Your urologist may offer guidance if you ask for a specific recommendation.
