I had my vasectomy six months ago and have just provided my 2nd sample for evaluation after 50 ejaculations. The sample came back positive.

This seems impossible to me that I could still have living sperm six months later, but he wants me to have 25 more ejaculations and get my sample tested again. Is it possible I still have some sperm left or am I that "1 in a 1000" that had the spontaneous reconnection?

It is still possible to have some sperm. You will want to ask your doctor whether there are a lot of active sperm still in the specimen. If so, it may be time to start thinking about a repeat vasectomy.

There are some cases that will take six to 12 months to clear out all sperm. If these are motile/live sperm after six months, then the possibility of re-connection or some other anatomical variation is likely.

Your vasectomy probably did not work. You need to decide if want to do it again. Unfortunately, this happens about every 2,000 times and I have certainly witnessed it a few times with my patients. However, if your samples show only a rare sperm cell here and there, you should be good to go. Best wishes to you!

Assuming that both vas were severed, occasionally it takes longer to completely clear the semen of sperm, and a few "stragglers" may persist. Eventually the count should reach zero. IF the count shows significant and persistent sperm, it may require a redo.
