I had a vasectomy 3 months ago. I continue to have scrotal pain and within the last week noticed some blood in my semen. Is this normal 3 months out?

I don't have any pain with urination or with ejaculation. If I wear boxers, my scrotum hurts worse than with tighter fitting briefs, but still hurts. The blood in my semen came out of nowhere and hadn't had any in my semen prior. I called my urologist who did the procedure, but stated this is normal, but from what I read online, blood 3 months out isn't.

No blood is not normal but it may not be coming from your epididymis. It is much more likely coming from your prostate and an exam should reveal if that is the case and that can be treated. The real answer to your question depends on a complete GU exam. Remember, even though you have had a vasectomy that does not mean that you will not have any problems in your urinary system from that point forward

Pain can linger off and on for 3-4 months. Hematospermia can occur in any man and may not be related to vasectomy at all. Often, it's insignificant in younger males.

Blood in the semen can be a "scary" finding, whether you have had a vasectomy or not. But, fortunately that blood is not associated with something bad in most cases. In fact, blood in the semen is a relatively normal finding after some vasectomies for the first few months.

Still, pain 3 months after a vasectomy is not normal and may benefit from further evaluation with an examination. Possible causes include inflammation +/- infection at the vasectomy site or the epididymis (a structure adjacent to the testicle responsible for helping the sperm mature). Antibiotics are sometimes necessary. If the pain is new and didn't start soon after the vasectomy, then bleeding as the reason for the pain is unlikely.

If the pain persists for many months with relief, even with various treatments such as anti-inflammatory medications (such as ibuprofen) and various antibiotics, the condition of "post vasectomy syndrome" may exist. In any case I would make a follow up appointment with your current or other doctor.
