Am I exhibiting the signs of post-vasectomy infection?

I'm 5 months post-no-scalpel vasectomy. In the last week, I’ve developed a testicle ache with occasional sharp, burning pain up the vas into the upper groin. It varies in intensity and responds well to heat or ibuprofen. I assumed congestion. The urologist I saw today never heard of that and suspects infection. He gave me Doxycycline even though the urine test was negative. I have no unusual swelling or redness or fever. Is this possible? Should I wait it out before taking antibiotics?

It is not unusual to have epididymitis after a vasectomy, at varying periods of time. For reasons we don't understand—epididymitis is not bacterial in nature—it will improve with antibiotics. I would take the antibiotics but also realize that there are changes in the vas deferens and testicles after the vasectomy that can manifest in symptoms as you describe. Usually, they are self-limited and resolve with time or a course of antibiotics and anti-inflammatories.
