My boyfriend had a vasectomy 20 years ago when he was 33. Is there a chance he can get a vasectomy reversal after all of these years?

Yes, you can have a vasectomy reversal. The success rate, however, decreases a small amount each year from the time the vasectomy was performed. The success rate would average around 30 percent.

Yes, but successful pregnancy becomes less likely in proportion to time that has transpired between the vasectomy and the reversal--your partner's age is a factor too. Conception rates begin to sharply decline on average for women over 37. Again, this is average and is due to a decrease in regularly expected monthly ovulation.

Yes. Results are somewhat less the longer the timespan from the vasectomy, but it is possible.

Yes, but the longer you're out from the vasectomy the lower the chances. Make sure to go to someone who does a moderate number of vasectomy reversals (vasovasostomies). The chances are less than 50% of getting sperm at 20 years.

His options are reversal or sperm extraction combined with IVF/ICSI.

Yes. He has about a 50/50 chance of pregnancy if you, his partner, are less than 38yo.

The answer is yes, but the pregnancy rates are lower (40% versus over 60%) after more than 15 years since the vasectomy because there is a higher chance he would need a connection to the epididymis (a vasoepididymostomy). In his situation, the reversal needs to be done by a very experienced microsurgeon who does a lot of vasoepididymostomies.

Yes, he can get his vasectomy reversed. There is a chance he may need an anastomosis from the vas to the epididymis due to long interval. You should find an experienced microsurgeon who can perform vast-epididymostomy, if its necessary.
