It's been 6 years after my vasectomy and I still feel like there is a stich remaining in my testicle.

A couple years ago the doctor said if it doesnt bother me I should leave it. It does become painful from time to time and also super sensitive. Could it be a stitch? What do you recommend?

In the area of the vasectomy site the body perceives it as trauma and as such the body reacts to it. This can result in a sperm granuloma or a scar that you might feel. As well if a clip was used this does not dissolve and it is something you can feel. As always if there's any concern consult your doctor. In general what you are feeling at this juncture is most probably nothing to be concerned about.

It could be a stitch or a clip. It could also be a sperm granuloma. If it bothers you, you need to be checked out.
