I had a vasectomy a year ago and have always had two lumps at the incision site, but one is larger than the other, roughly a pea or slightly larger.

when they did that side it was pretty painful so I think it maybe scar tissue or this sperm granuloma. It doesnt hurt at all and has changed a little in shape over the year but not much. Is it what I think and is it here to stay? Just curious.

Hi and you are most likely right - the tiny lumps that you feel being a sperm granuloma or just scar tissue around the site of the vasectomy. If they are not uncomfortable in any way, I would just leave them alone. It's very common actually. Nothing to worry about.

You are correct. The lumps are at the site of the vasectomy on each side. It is not an uncommon finding and if they do not hurt there is nothing to do. It is also not uncommon for one side to be different from the other. The body treats sperm as a foreign body so the lumps are an inflammatory response. I had a friend who had this occur and he called them his "sperm granolas."
