Vasectomy when 28. 30 years later, my epididymitis is the same size as my testicle.

I don't know exactly when it happened but about 10 years ago, I noticed my epididymis was the size of my testicle. It didn't happen gradually, either. One day I just noticed it was large. Ever since my vasectomy, I had some problems with the left epididymis, it was sorer longer than the right after operation. From memory it has always been a little tender, felt solid and a little larger than the right side. So after 30 years, all at once it got big and has stayed that way for the last 10 years.

That long after a vasectomy makes it unlikely that it is anything related to the vasectomy at all. You would need a complete evaluation perhaps using ultrasound to see if there is anything abnormal that is occurring.

Probably have a spermatocele, which is a cystic collection of sperm due to a leakage from the epididymis.

If this mass is at the superior aspect of the testicle and the size you mention it is a spermatocele. You can take a flash light in a darkened room and shine the mass. If it illuminates you've got your diagnosis. Having said this, any mass of the scrotum should be evaluated by your urologist who will probably confirm with a scrotal ultrasound. The spermatocele may or may not be related to having had the vasectomy as we see these commonly in non vasectomy patients. It requires treatment only if it is symptomatic. Look up the anatomy of the testicle where it attaches to the epididymis and you'll see the Ductuli Efferentes. It is here where the spermatocele forms.

Most likely you have developed epididymal cysts resulting in the enlargement. It is a common and benign condition.
