It has been 10 months since his vasectomy. He's done two analyses and was told to do another one in two months time. Does he still need to use contraception?
I would say yes, although the chances of conception with that number of sperm is very unlikely. Next time you give a specimen, ask them to actually count the number of sperm per high-powered field.
I see post-vasectomy semen commonly that have 1 or 2 dead sperm. This is a conundrum because everyone would prefer to see "zero." So, this becomes a personal decision.
I personally would advise repeating until it is negative completely, although there are some who it never is complete and they consider a second vasectomy. This is a tough situation for the patient and the doctor, both of which want no part of an unwanted pregnancy.

If both of these semen analyses were processed fresh within 2 hours of ejaculation, then contraception is no longer required.
The American Urologic Association defines vasectomy success as examination of one well-mixed, uncentrifuged fresh post-vasectomy semen specimen demonstrating azoospermia (absence of sperm) or only rare non-motile sperm (defined as less than 100,000 sperm / mL). Yes, he may have a minute connection even though he is quite subfertile. In my practice I would offer a repeat vasectomy in this case
